The reason is usually not discipline: it is that the click gets switched on at the target tempo, the passage falls apart, and the conclusion drawn is "I am bad at timing" rather than "the tempo was wrong". A metronome is a measuring instrument. It tells you where you are; it does not carry you.

Tempo: the number you set, and the number you should have set
The single most common mistake is starting near the target. If a passage is written at 132 and you can only play it cleanly at 92, then 92 is the tempo of that passage today. The guide below is the working range for each tempo marking, and the reason the traditional Italian words are still useful: they describe a character, not just a speed.

BPM is a duration, not a speed
One quarter note lasts 60000 / BPM milliseconds. At 120 BPM that is 500 ms; an eighth is 250 ms, a dotted eighth 375 ms, an eighth triplet 166.7 ms. This matters in two places. In practice, it tells you how much time you really have between notes — a run that "feels impossible" at 160 is asking for 93 ms per sixteenth, which is a physical fact you can plan around. In production, it is how you set a delay so the repeats land on the grid instead of smearing.

Meter: what the click is counting
A time signature is not decoration. It says how many pulses a bar has and which of them are strong, and that grouping is what makes 7/8 sound like a groove rather than seven identical clicks. Simple meters divide each beat in two; compound meters (6/8, 9/8, 12/8) divide each beat in three; odd meters (5/4, 7/8, 11/8) are counted in unequal groups — 3+2, 3+2+2, 3+3+3+2.
The practical consequence: in 6/8 you do not count six. You feel two dotted-quarter pulses and let three eighths live inside each. Set the metronome to the dotted quarter for the main pulse, and to eighths only when you are drilling the subdivision. The explorer below plays each meter with its accents in place.
Meter
Meter is a rhythmic pattern of beats (measures) that gives music its structure and tempo. Different meters have different accent patterns and numbers of beats per measure. Select a meter to see more information about it.

Four drills that are worth the hour
Most metronome practice is one drill — play the thing at increasing speeds — done badly. These four attack different problems, and only one of them is about speed.
- Step up in fives. Three clean repetitions at a tempo, then +5 BPM. One mistake and you drop back a step. This is slower than it sounds and it is the only method that reliably holds.
- Click on 2 and 4. Move the accent off the downbeat and your own sense of "one" has to hold the bar. It exposes rushing instantly, and it is how jazz and funk players actually practise.
- Subdivide, then stop subdividing. Set the click to eighths or sixteenths until the passage locks, then take it back to quarters. Your internal grid keeps the resolution; the click stops doing the work.
- Drop the click. Play four bars with it and four without, alternating. If you are a beat early coming back in, you are rushing — which no amount of playing along to a click will tell you.

Beyond one pulse: polyrhythm, cross rhythm, metric modulation
Once one click is comfortable, the interesting problems start. A polyrhythm is two pulses of different length over the same bar — 3 over 4 — where neither is a subdivision of the other. A cross rhythm is that used as the groove itself. Metric modulation re-hears an existing subdivision as the new pulse: an eighth-note triplet becomes the new eighth, and 120 BPM becomes 180 without a single note changing speed.
